Bringing Health Care Home
Since 1973, Adventist Home Health has provided comprehensive skilled services and compassionate care to help patients recover and function as independently as possible in their own homes.
Our home care program is designed to:
- Help you and your family or caregiver in following the medical plan of care
- Educate you and your family or caregiver to properly use equipment or perform procedures at home
- Support you and your family or caregiver to follow your medication regimen and rehabilitation program
- Communicate your progress with your physician
- Encourage compliance with medical appointments
- Decrease the chance of your return to the hospital or emergency room
To be eligible for home care, you must:
- Have a medical condition that requires the skill of a licensed professional on an intermittent, part-time basis
- Be under the continuous care of a physician who is willing to work with a home health services provider
- Be homebound as required by Medicare and some insurance policies
Accredited by the Community Health Accreditation Program (CHAP), Adventist Home Health is a state-licensed home health agency and is certified by Medicare. Most insurances will pay if criteria is met.
